Shelburne Family Practice is a collaborative primary care clinic dedicated to comprehensive, patient-centered healthcare. The team includes family physicians, a nurse practitioner, a family practice nurse, a dietitian, a social worker, and clerical staff. Together, they provide full-scope primary care, chronic disease management, preventative health, mental health support, nutritional counseling, and administrative coordination.
