Company Description
Nova Scotia Health (NSH) is embarking on an innovative model of care in collaboration with Shannex to provide high quality care for patients transitioning from acute care and into community settings. Reporting to the Medical Site Lead – Transitional Care, the physician will function as part of a collaborative multidisciplinary team to provide medical care to all patients admitted to West Bedford Transitional Care Facility.
We seek physicians who will model excellence in general medical care, have superb communication skills with patients and families, and who work well in team settings. There will be a high focus of patient flow through the facility. As this is not a long-term care facility, physicians will work collaboratively with Nurse Practitioners and the larger multidisciplinary team to flow patients to community daily.

Responsibilities
- Abide by NSH Medical staff By-Laws, NSH policies, procedures, rules, and regulations
- Accountability to the Medical Director-Transitional Care and to their CZ Department Head, ultimately to the CZ Medical Executive Director
- Provide direct inpatient care for patients admitted to the transitional care facility
- Collaborate with the Transitional Care Team, community practitioners, acute inpatient teams and/or emergency department teams as needed, determined by the patients’ plan of care and disposition
- Provide most responsible health care provider (MRHCP) coverage for admitted patients, ensuring all laboratory, imaging consultation and other reports are received and reviewed daily
- Equitable participation in after-hours call coverage
- Daily participation in flow discussions and multidisciplinary team meetings
- Embrace the Home First Philosophy of NS Health in efficient discharge planning with focus on expected length of stay with patient/caregivers at time of admission to the facility
- Participate in quality improvement, operational and research initiatives as required
- Attend site-based physician and/or staff meetings
- Be comfortable providing short term acute care for sick patients who decompensate and require escalation/transfer to acute care
- Demonstrate commitment to delivering the highest quality care and maintenance of competence
- Demonstrate an interest in teaching
Qualifications
- All applications must be eligible for licensure with the College of Physicians and Surgeons of Nova Scotia (CPSNS); all specialists must be eligible for certification with the Royal College of Physicians and Surgeons of Canada (RCPSC)
- All applicants must have membership with the Canadian Medical Protective Association (CMPA)
- Central Zone active with admitting privileges via NSH Credentialing Committee are required
- Experience in acute care and post-acute care is considered an asset

Remuneration:
Remuneration is sessional for weekday work. Evening/weekend/holiday call is paid as per the Category 1 Facility On Call Fee, as per the current Master Agreement.
Expected Hours
- Flexible schedule, most often 1-week blocks
- Daytime hours 0800-1700h (paid sessionally)
- Weeknight call 1700-0800h (paid via facility on call fee)
- Weekend/holiday call 0800-0800h (paid via facility on call fee)
- Suitable participation in overnight and weekend/holiday coverage is required
Community Details:
Bedford is a suburban community of Halifax, Nova Scotia, Canada. It was an independent town from 1980 to 1996. Bedford is on the northwestern end of Bedford Basin, an extension of the Halifax Harbour, which ends just before Nova Scotia Highway 102 and the Bedford Bypass, next to Lower Sackville. There is a popular walkway along much of the Bedford Basin waterfront that begins at DeWolf park, and continues as the Bedford-Sackville Connector Greenway, a crushed gravel covered trail that meanders along the Sackville River. There is an outdoor 25m pool and smaller splash pool located at the Lions Park on Shore Drive. In the summer there are numerous lakes suitable for swimming. The topography of the area limits the possible locations for soccer and football fields. The community is currently home to six ice surfaces. Bedford is a well-established sailing community, and is home to the Bedford Basin Yacht Club and Marina.
