About the Opportunity

Nova Scotia Health (NSH) is seeking expressions of interest for the position of Medical Director/Site Lead Transitional Care Hogan Court/Stratford Transitional Care Site within the Central Zone (CZ).

The Medical Director/Site Lead – Transitional Care will provide medical/clinical leadership in the care of NSH patients admitted to two transitional care facilities in CZ, Hogan Court and Stratford Hall, a collaborative care model established between Nova Scotia Health (NSH) and Shannex. Patients at both facilities are designated as post-acute or Alternative Level of Care (ALC).

Position Summary

Mandate and Accountability:

Functioning in a co-leadership model working with NSH and Shannex administrative co-leads, the Medical Director/Site Lead provides clinical leadership. The Medical Site Lead/Director provides leadership to members of the health team to ensure quality service provision in accordance with the NSH Medical Staff By-laws, NSH Rules and Regulations, policies, and procedures, and accepted and evolving standards of clinical practice. As required, the physician leader will be expected to enable appropriate and timely transition of patients into and out of Transitional Care Facilities (TCFs), back to community, in accordance with organizational, contractual, team, patient and family/caregiver goals. The Position Reports directly to the Central Zone Medical Executive Director (ZMED) with shared accountability to the Central Zone Vice President Operations (VPO).

Responsibilities:

  • Participate in the implementation of the NSHA’s strategic priorities/plan at CZ TCFs.
  • Develop and implement care escalation pathways to appropriately manage the transfer of patients between the TCFs; community; acute care facilities or other facilities across the province, if applicable
  • Provide leadership and operational support, as required on issues that arise within the site(s) including but limited to access & flow, quality of care, space planning, disaster and emergency preparedness
  • Participate in space allocation decisions and sign-off of any service changes to the site(s) (i.e. bed closure, service reductions, etc.)
  • Provide a critical liaison between the clinical staff, operational staff and physicians.
  • Support positive public relations within the site(s) and surrounding communities in cooperation with NSH Public Relations and Shannex Strategy and Brand team
  • Provide leadership in promoting and providing service within the Quality Framework while adhering to Values, Ethics, Patient & Occupational Safety philosophies and practices.
  • Co-chair a site-based Clinical Affairs Committee and lead clinical quality related initiatives at the site(s) as required.
  • Ensure that the ZMED and VPO are aware of the perspectives of MRHCP at the applicable site(s) and brings information relevant to the site(s) forward for consideration during the development and implementation of strategic priorities/plans.
  • Support the development and wellness of the MRHCP, clinicians, volunteers, and caregivers within the site(s)/local area.
  • Enable, facilitate, and ensure effective team communications and functions.
  • Promotes Home First Philosophy
  • Provide leadership in fostering a culture of safety for patients/clients, staff, clinicians, volunteers, medical staff, and public safety.
  • Support and contribute to site policies and systems related to emergency planning/response, quality monitoring and improvement, service utilization, risk management, occupational health and safety.
  • Provide clinical leadership in identifying, assessing, and anticipating future requirements, problems, directions and development for all aspects of Occupational Health and Safety, Fire Safety, Emergency Management, Workplace Violence and Prevention programs at site(s) and District levels.
  • Actively assist in managing site overcapacity issues by being the point of contact for the clinical staff.

Qualifications

We are seeking a physician who:

  • Currently holds NSHA privileges 
  • MD(with or without specialty) with evidence of past and ongoing leadership training
  • 3 - 5 years as a medical department member with experience engaging, mentoring, and empowering teams and stakeholders in a collaborative, participative manner
  • Experience with co-leadership is considered an asset.
  • Demonstrates expertise in the areas of:
  • access and flow 
  • system accountability
  • quality and patient safety
  • evaluation
  • resource utilization
  • team building and collaboration

Additional Information

Compensation: 

$30,000.00 per year  and estimated to be 0.1-0.2 FTE
