About the Opportunity

The Diabetes Centre is a community-based program that provides patients, and their families, with the support they need to manage their Type 1 and Type 2 diabetes. The goal of the Diabetes Centres is to provide education and self-management support for patients to manage their blood sugars, cholesterol levels, and blood pressure. The Diabetes Centres assist patients to maintain their health and prevent the complications of diabetes in a model that follows individual and group programming, both in person and virtually. The Diabetes Centres support an interdisciplinary and integrated approach to diabetes education. Patient-centred care, behaviour change, and self-management are core to all programming and services.

 

Under the supervision of the Manager of Primary Health Care, the Pharmacist will work with the interdisciplinary team and other dedicated front-line pharmacy staff to provide patient-centred care that optimizes safe, effective, and appropriate drug therapy. This includes identifying and resolving drug-related problems, completing medication reconciliation, participating in team care planning, providing patient education, and making therapeutic recommendations to the medical team and community primary care providers. The Pharmacist is also responsible to ensure safe and effective drug therapy and contributing to the effective and efficient attainment of the goals and objectives of Nova Scotia Health Pharmacy Services.
 

This position will support Diabetes Centres across the zone with occasional travel.

About You

We would love to hear from you if you have the following:

  • Graduation from an accredited university program in Pharmacy
  • Completion of a Pharmacy Residency Program preferred
  • Licensed to practice Pharmacy in the Province of Nova Scotia
  • Minimum two years' experience in clinical practice in a hospital pharmacy setting preferred
  • Strong knowledge of drugs and therapeutics
  • Effective oral and written communication skills
  • Ability to make accurate and timely recommendations regarding safe and cost-effective use of drugs
  • Competencies in other languages an asset, French preferred 
  • Valid Nova Scotia Driver's License and access to a reliable vehicle

Hours

  • Long Assignment, Full-time position; 75 hours bi-weekly
  • Beginning August 25, 2025 with a duration of October 2, 2026
  • Dates are subject to change
  • Shits may include evenings, weekends & holidays
  • This role will require the successful candidate to work from both the Mumford Professional Centre and Spryfield Community Centre, as required

Compensation and Benefits

$49.42 - $61.74 hourly
