About the Opportunity
Reporting to the Manager Care Coordination, the Team Leader provides leadership, innovation, and support with assessment, authorization, service planning and collaborative case management aligned with provincial policies for continuing care programs and services. The team leader works closely with continuing care staff and managers, contracted home support, nursing and long-term care service providers, NSH clinical programs and team members, primary healthcare providers along with community agencies and organizations, to promote person-centered care. Roles and responsibilities may include:
- Fostering healthy team processes that develop a person-centered continuum of care, applying the LEADS framework in a health care environment
- Facilitating collaboration between respective program areas and their networks across the continuum of care while linking with patients, families and community organizations
- Assisting in program planning and evaluation, ensuring practice is evidence-informed
- Participating in the orientation and mentorship of new team members
- Supporting a quality approach to service delivery
- Assisting with the development of the consistent application of policies related to continuing care programs and services
- Facilitate a culture of learning and excellence for staff and students
This role encompasses a portion of a pre-existing Care Coordinator position. 60% of this position will require the successful candidate to work as part of a team with clients, peers and other health care professionals while exercising team leadership functions. The remaining 40% of the work time spent will remain in direct client service.
About You
We would love to hear from you if you have the following:
- Currently employed as a Care Coordinator of the NSH Continuing Care Service
- Bachelor’s degree in Social Work, Occupational Therapy, Physiotherapy, Nutritional Studies or Nursing (diploma in nursing may also be considered)
- Minimum five (5) years related experience
- Current registration (or eligibility) with applicable provincial licensing body required
- Demonstrated leadership capabilities
- Ability to conduct an extensive comprehensive multi-dimensional assessment in client’s home
- Demonstrated experience developing, monitoring and evaluating complex care plans that respond to client’s changing health needs
- Ability to integrate existing service options with a non-traditional health care approach when developing care plans
- Ability to develop relationships, work collaboratively and effectively with clients, families and health care professionals
- Ability to work within a changing environment and must demonstrate flexibility and a willingness to take on new assignments
- Organizational and prioritization skills and the ability to manage multiple tasks
- Demonstrated excellence in problem solving, interpersonal and written/verbal communication skills
- Demonstrated proficiency in computer applications including Microsoft Office products i.e. Word, Excel, Outlook, PowerPoint required
- Possess a valid driver’s license and access to a reliable vehicle as travel within the health authority is required
- Competencies in other languages an asset; French preferred

Compensation and Benefits
$42.56 - 48.27 Hourly
